Release Form Policy

• All submissions for the newsletter and website must include a signed copy of the Release Form to permit the publication of personal information.
  
  
• All photographs submitted for publication must include a Release Form for each person in the photograph.
  
  
• Electronic copies of documents and photographs are preferred.
  
  
• Completed forms may be scanned and e-mailed to the Webmaster.
